- If you were unable to pick up a document at OIS from before March 13 and would like it mailed to you:
- Please send an email to ois@usc.edu with the subject line: MAIL MY DOCUMENT
- Include your name, USC Student ID #, SEVIS #, date of birth, type of document, and preferred mailing address
- Your document will be mailed via USPS on or before April 10, 2020. Please allow four business days delivery for domestic addresses and two weeks for international addresses
- For all other document processing requests (i.e. travel signature, program extension, reduced course load, leave of absence, letter requests, etc.): Complete the appropriate OIS Request Form and email it to ois@usc.edu
- Subject Line of email should identify Type of Application: Name of Student, SEVIS #
- Example: Travel Signature: Tommy Trojan, N0012345678
- Example: Program Extension, Tammy Traveler, N0012345678
- Include your preferred mailing address in the body of the email
